Behavioural/Neurodivergent/ADHD/ASD – how to book an appointment- Help us to help you!

To secure your behavioural appointment with one of our paediatricians, please send the following documents in ONE email to [email protected]; separate emails will NOT be actioned:

1. Two completed SNAP-IV forms (parent and teacher)

2. A current GP referral

3. Completed new patient form

4. Your child’s two most recent school reports

5. A short family history (100 words or less) outlining neurodiverse conditions, saved as a Word document

Please also attached any additional specialist reports, such as psychology, speech pathology or OT assessments on this email, if applicable. We understand this may be your first appointment to investigate your concerns, so you may not have these.

Once we receive the complete file, we aim to contact you within 1 week to discuss next steps. If you have questions or need help, please reach out. We cannot proceed without all documents in one email, and your cooperation is appreciated to ensure a smooth booking process. If you’ve already sent everything and haven’t heard from us within a week, please let us know.

For ease of completion, please download and fill out the forms on a computer if possible. Photos of documents or reports will NOT be accepted; please ensure they are scanned before sending.

Paediatric Initial Standard: $427 ($151.90 Medicare rebate)
Paediatric Follow-up Complex: $296 ($133.00 Medicare rebate)
Paediatric Follow-up Standard: $196 ($76 Medicare rebate)
DCJ Paediatric Initial Complex: $703 ($265.60 Medicare rebate)
